About The Position
As a Spiritual Care Coordinator at OnCare Health, you will play a vital role in providing compassionate spiritual support to patients, families, and caregivers within our hospice program. Your primary responsibility will be to conduct thorough spiritual assessments, ensuring that the unique spiritual needs of each individual are recognized and addressed. You will work closely with the hospice team to facilitate the development of individualized care plans that reflect these needs, ensuring that appropriate hospice services are delivered in a timely manner. In addition to assessments, you will maintain detailed records of spiritual care services and related activities. This documentation is crucial for quality assurance, program development, and the ongoing review and revision of policies and procedures. Your role will also involve collaborating with team members to plan and execute periodic memorial services, which are essential for supporting personnel, volunteers, and community spiritual counselors involved in the hospice care process. Building strong relationships within a positive team environment is key to our mission at OnCare Health.
