About The Position

The Spiritual Care and Bereavement Coordinator at Devotion Hospice, LLC is responsible for providing spiritual care and counseling to patients, family members, and caregivers, ensuring alignment with their beliefs and the patient's care plan. This role also involves coordinating bereavement support for families during hospice care and up to 13 months post-passing, integrating these services into the overall care plan in collaboration with the interdisciplinary team.

Requirements

  • Ordained, commissioned, or credentialed according to the practices of an organized religious group or a degree in theology or from a seminary.
  • Experience in interfaith settings with sensitivity to diverse spiritual practices.
  • Experience in bereavement support or counseling.
  • Hospice or palliative care experience.
  • Ability to support diverse cultural, religious, and lifestyle preferences.
  • Valid driver's license and access to a reliable, insured vehicle.
  • Strong communication skills, both verbal and written.

Nice To Haves

  • 1 year of hospice experience (Preferred)
  • 1 year of bereavement support experience (Preferred)
  • Ability to commute to Novi, MI (Preferred)

Responsibilities

  • Conduct a spiritual assessment within 5 days of patient admission.
  • Assist in developing and implementing the interdisciplinary plan of care related to spiritual care.
  • Provide support to patients and families according to their religious preferences or definitions of spirituality.
  • Maintain and submit accurate clinical records, including assessments and outcomes of spiritual care.
  • Provide an initial bereavement assessment at admission and for the family within 2 weeks of the patient's death.
  • Develop and maintain a bereavement plan of care tailored to grieving families and caregivers.
  • Offer individual and group bereavement counseling and support as appropriate.
  • Facilitate bereavement workshops, support groups, and memorial services.
  • Maintain accurate records of bereavement interactions and provide reports as needed.
  • Participate in QAPI meetings and provide input for improving bereavement services.
  • Consult and educate the interdisciplinary group on spiritual care and bereavement issues.
  • Offer funeral or memorial services as requested by the family.
  • Ensure all services respect the diverse values, beliefs, and practices of patients and families.
  • Represent Devotion Hospice with professionalism and compassion in the community.
  • Participate in professional and educational opportunities for self-development.
