Job Summary: The Speech Therapist provides treatment to the patient to improve sensory-motor development, perception, memory. language and cognition.

Requirements

  • Knowledge of speech therapy practice, with emphasis on the provision of physical therapy services in the home environment.
  • Ability to foster constructive and therapeutic relationships with patients/clients and their families.
  • Ability to communicate effectively, both orally and in writing.
  • Licensed as a Speech Pathologist
  • Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association.
  • Two years’ experience as a Speech Therapist.
  • Current, valid license to practice as a Speech Pathologist.
  • CPR certification.
  • Current, valid driver's license.
  • Access to a reliable automobile

Nice To Haves

  • Previous experience in the home care or outpatient therapy industry preferred.

Responsibilities

  • Provides initial and ongoing interdisciplinary assessment of the patient to identify the patient’s level of functioning, swallowing and communication ability.
  • Provides speech therapy in compliance with practitioner orders, patient’s plan of care, FLHC policies and procedures and federal and state guidelines
  • Develops a plan of care in consultation with the health care practitioner and other team care members.
  • Instructs patients and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
  • Instructs patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language and cognition.
  • Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age.
  • Documents all care and services provided according to professional standards and agency policy and procedures.
  • Adheres to all agency policies and procedures.
  • Completes all mandatory agency training and in-service activities.
  • Participates in the agency’s quality management/QAPI activities.
