About The Position

Home Health Speech Therapist (SLP) We are seeking a compassionate and motivated Home Health Speech Therapist (Speech-Language Pathologist – SLP) to join our team. This role provides skilled speech therapy services to patients in their homes, focusing on improving communication, cognition, voice, and swallowing to enhance safety, independence, and quality of life. The Speech Therapist evaluates patient needs, develops individualized plans of care, and delivers evidence-based treatment in accordance with physician orders, agency policies, and regulatory standards.

Requirements

  • Graduate of an accredited Speech-Language Pathology program
  • Current Speech-Language Pathologist (SLP) license in the state of practice
  • Certificate of Clinical Competence (CCC-SLP) preferred or required per state regulations
  • Minimum of one (1) year of Speech Therapy experience; home health preferred
  • Knowledge of home health regulations and documentation requirements
  • Strong clinical assessment and organizational skills
  • Excellent verbal and written communication skills
  • Ability to work independently and manage a flexible schedule
  • Ability to legally work in the United States
  • Ability to travel between patient homes
  • Ability to assist patients with positioning and therapeutic activities as needed
  • Ability to stand, walk, bend, and reach during patient care
  • Ability to communicate effectively with patients, families, and healthcare team members

Nice To Haves

  • Certificate of Clinical Competence (CCC-SLP) preferred or required per state regulations
  • Minimum of one (1) year of Speech Therapy experience; home health preferred
  • Knowledge of home health regulations and documentation requirements

Responsibilities

  • Evaluate patients to identify speech, language, cognitive, voice, and swallowing disorders
  • Develop and implement individualized speech therapy plans of care
  • Provide skilled speech therapy treatment in the home setting
  • Treat conditions related to communication, cognition, and dysphagia
  • Educate patients and caregivers on treatment plans, strategies, and safety techniques
  • Reassess patient progress and modify treatment plans as needed
  • Coordinate care with the interdisciplinary healthcare team
  • Participate in discharge planning and discharge patients when goals are met
  • Maintain accurate, timely, and compliant clinical documentation
  • Adhere to professional, ethical, and regulatory standards of practice
