You will work collaboratively with physicians, patients, and caregivers to develop and implement individualized plans of care that promote recovery, safety, and independence. Your expertise will directly impact each patient’s ability to function at their highest potential through evaluation, treatment, education, and ongoing support. Requirements

Current Speech Therapist license (State of Florida required)
Commitment to the mission and values of Haven Home Health

Nice To Haves

Minimum of two (2) years of Home Health experience preferred
Strong clinical judgment and ability to work independently

Responsibilities

Perform initial evaluations and develop individualized treatment plans
Provide skilled speech therapy services as ordered by the physician
Assess, treat, and educate patients with communication and swallowing disorders
Collaborate with interdisciplinary team members to coordinate patient care
Participate in case conferences and care planning
Monitor patient progress and adjust treatment plans as needed
Complete timely and accurate clinical documentation, including OASIS assessments
Ensure compliance with agency policies and regulatory standards
