About The Position

Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. Who You Are: You are a motivated Speech Therapist who shares our vision of delivering home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment. Travel : This role comprises 100% travel within our service region. You will be traveling to patients’ homes. Service Region: We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding areas such as Colusa, Glenn and Tehama. Schedule : This is a Part-time role, with the possibility of expanded hours in the future.

Requirements

  • Master’s degree in Speech-Language Pathology from an accredited program
  • Active California SLP license
  • Current CPR certification
  • Minimum of 1 years’ experience, preferably in home health or medical setting
  • Valid driver’s license, reliable transportation, and willingness to travel
  • Eligible to be insured under our liability policy with a clean DMV report
  • Excellent communication, interpersonal and time management skills
  • Ability to work independently and as part of a multidisciplinary team
  • Conditional job offer is successfully passing a medical exam.
  • Physical requirement for this role specifically includes Sits, stands, bends, lifts, walks, and moves intermittently during working hours.
  • Able to lift 50 pounds without assistance.

Responsibilities

  • Conduct in-home assessments and speech-language evaluations
  • Develop and implement individualized treatment plans
  • Provide therapy focused on communication, cognition, voice, and swallowing disorders
  • Educate patients and caregivers on exercises and communication strategies
  • Maintain clear and timely documentation in accordance with home health regulations
  • Collaborate with nurses, physical/occupational therapists, and physicians as part of an interdisciplinary team.
