About The Position

Benchmark Therapy and Rehab is a large therapy practice providing services to a multitude of providers including Home Health. They are seeking compassionate and driven Speech Language Pathologists/CCC-SLPs to join their therapy team in Ohio for their Dayton Practice. Benchmark Therapy provides therapy in geriatric healthcare in the home settings, aiming to produce measurable outcomes through therapy interventions. The company offers an exclusive "FOCUSED SPEECH REHAB" program and strives to provide the best care for its patients. The position involves providing speech therapy services to patients in a home health setting by assessing patient needs and generating treatment programs with established goals.

Requirements

  • Must be a certified Speech Language Pathologist (CCC-SLP).
  • Ability to complete performance requirements independently while using good judgment.
  • Ability to harmoniously work with others.
  • Adherence to all policies of the company, including job description, certification requirements, mission statement, HIPAA, confidentiality standards, and resident rights.
  • Ability to adapt to changes in responsibilities in a positive manner.
  • Receptiveness to new ways of completing tasks.
  • Flexibility in carrying out assignments.
  • Ability to communicate effectively and professionally while maintaining patient confidentiality when corresponding with patients, patients' family, and other healthcare professionals.

Responsibilities

  • Provide speech therapy services to patients and initiate referrals when appropriate.
  • Review physician's referral (order), patient's condition, and medical records to determine that speech therapy treatment is required.
  • Evaluate patients to assess patient needs, conduct formal/informal assessments necessary for treatment planning and implementation.
  • Conduct specialized evaluations as indicated.
  • Plan and implement individualized treatment programs by identifying a target list, including short and long term goals and methods to achieve identified goals.
  • Document results of patient's assessment, treatment, follow up and termination of services.
  • Provide treatment in accordance with established standards of practice, department procedures, and productivity standards.
  • Consistently demonstrate sound judgment in evaluation, planning implementation, and follow-up of resident therapy programs.
