About The Position

We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full-Time or Part-Time Speech Therapist (ST) in Alameda , CA. We are seeking an experienced, self-driven Full-Time or Part-Time Speech Therapist (ST) to join our Home Health team and provide exceptional care throughout the region. As a ST, you will assess patients’ speech and communication needs, develop individualized treatment plans, and support their progress to enhance their quality of life. You’ll collaborate closely with our Clinical Director and Clinical Staff, working independently in the field and remotely. Our dedicated back office of CA Licensed and experienced clinicians is here to support you while you’re in the field. Whether you need assistance with a new situation, help communicating with a patient if you’re running late, or someone to call in lab results, you’ll always have reliable support when you’re part of our team. We provide industry-leading tools to streamline your daily operations.

Requirements

  • Current OT License to practice in California - Required
  • Experience in Home Health or Hospice care - 1 Year Required
  • Current CA Driver’s License
  • Able to travel within a 10 - 15 mile radius on a regular basis.

Responsibilities

  • Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team.
  • Conduct initial and ongoing evaluations of patient status.
  • Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
  • Deliver educational information to patients and their families regarding speech and language disorders and treatment plans.
  • Maintain accurate clinical records, submitting documentation in a timely manner per company policy.
  • Adhere to all company policies and procedures to ensure the highest standards of care.
