About The Position
At Interim Healthcare, we are committed to providing compassionate, patient-centered care that prioritizes dignity and quality of life. We are seeking a dedicated Speech Therapist to support our hospice patients with communication and swallowing needs in a home-based setting.
Requirements
- Licensed Speech-Language Pathologist (SLP) in Tennessee
- Experience in hospice, home health, geriatrics, or skilled nursing preferred
- Strong knowledge of dysphagia management and communication disorders
- Compassionate, patient-centered approach
- Excellent communication and teamwork skills
- Ability to work independently in a home-based setting
Responsibilities
- Evaluate patients for speech, language, cognitive, and swallowing disorders
- Develop individualized care plans focused on comfort, safety, and quality of life
- Provide interventions to support communication, cognition, and safe swallowing
- Educate patients, families, and caregivers on techniques and compensatory strategies
- Recommend diet modifications and adaptive equipment as appropriate
- Collaborate with the hospice interdisciplinary team (nursing, social work, chaplain, aides, physicians)
- Accurately document patient care and progress
