About The Position

A home health program serving the Lewisville area is seeking a Speech Therapist (SLP) to provide patient-centered speech, language, cognitive, and swallowing therapy in the home setting. This PRN opportunity offers autonomy, flexible scheduling, and consistent referrals throughout Lewisville and surrounding communities.

Requirements

• Active Texas Speech-Language Pathologist (SLP) license.
• Reliable transportation and ability to travel for home visits.

Responsibilities

• Complete in-home speech therapy evaluations and develop individualized treatment plans.
• Deliver therapeutic interventions aligned with clinical standards and patient goals.
• Document progress, plan updates, and visit outcomes in accordance with home health and regulatory guidelines.
• Coordinate care with interdisciplinary team members, families, and caregivers.
• Provide education on communication strategies, swallowing precautions, and home exercises.
• Maintain compliance with all professional and home health practice requirements.
