About The Position

Requirements

• Master's degree in Speech-Language Pathology or Communication Disorders (may be MA, MS, Med)
• New York State licensure or for recent graduates: Completion of at least 9 months supervised Clinical Fellowship year OR eligible to participate in supervised Clinical Fellowship year by a NYS licensed Speech Language Pathologist with a NYS Limited permit and followed by the NYS licensure examination and achievement of NYS license

Nice To Haves

• 1 year experience in medical setting preferred
• For Long Term Care/Elderone: 1 year experience with frail elderly population preferred

Responsibilities

• Evaluation & Planning. Perform a complete initial evaluation of referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence
• Documentation. Maintain a complete documentation of patient evaluations, treatment, progress and discharge status; send required information to appropriate professionals
• Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals and assisting in discharge planning as appropriate
