The Speech-Language Pathologist organizes and conducts the Speech-Language Pathology plan of care to facilitate the habilitation or rehabilitation of individuals with speech, language, cognitive-linguistic, psychosocial, and swallowing disabilities. This role involves directing patient participation in selected activities aimed at establishing, enhancing, and restoring these abilities. The Speech-Language Pathologist performs evaluations, develops effective treatment plans tailored to each patient’s rehabilitation needs within the care continuum, and initiates and participates in the discharge planning process. Additionally, the position requires educating patients, families, and caregivers on relevant health information, treatment, and prevention strategies. Other departmental activities include documentation, billing, and supporting department operations. The Speech-Language Pathologist must comply with all policies and standards, maintain the confidentiality of Protected Health Information (PHI), and complete annual training as required.
