Requirements

  • Speech Therapist with a current license in the state of employment.
  • Minimum one-year experience as a ST in an acute care setting.
  • Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.

Responsibilities

  • Develop an individualized ST plan of care and makes adjustments as needs change.
  • Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.
  • Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
  • Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  • Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
