JOB SUMMARY At NorthBay Health the Speech Language Pathologist in accordance with his/her evaluation findings and the physician’s referral, plans and implements specific treatment programs for individual patients according to the principles and practices of speech-language pathology. Collaborates with the interdisciplinary treatment team. Inpatient SLPs may work occasionally in the high risk infancy clinic and NICU and throughout the hospital system with adult patients. PRIMARY JOB DUTIES Provides speech-language pathology services in inpatient and outpatient settings during assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage. On physician’s referral, evaluates patients, develops specific treatment plans and goals, administers procedures and modalities specific to the learning needs and age of the patient, and plans for discharge utilizing appropriate Speech-Language Pathology techniques. Acts as a case manager for assigned patients. Prepares and maintains accurate and current patient records in accordance with departmental policies and procedures. Re-evaluates and revises patient’s treatment plan on a regular basis, with approval of physician as necessary. Provides patient, family/caregivers with age appropriate instruction regarding therapy procedures and home program, and provides additional instruction as indicated. Instructs the patient and family/caregiver in care and use of assistive and adaptive equipment. Communicates with rehabilitation team members, family, physicians and third party payers regarding patient progress and program changes as appropriate. Attends and/or participates inter/intradepartmental in-services and meetings as required and appropriate. Participates in short and long-term program development, including participation on committees as requested. Participates in student intern and/or volunteer program as appropriate. Provides supervision to CFY as appropriate. Maintains clean, safe and orderly treatment areas and equipment. Reports to supervisor any problems or suggestions regarding safety in the workplace. Assists with orientation and supervision of other health care professionals as appropriate. Establishes and maintains effective communication with physicians, co-workers, patients and other hospital personnel. Enhances professional growth and development through participation in educational programs, in-service meetings, and workshops within assigned specialty areas. Performs other duties as assigned.
