Greenfield Rehabilitation Agency is seeking a PRN Speech-Language Pathologist (SLP) to support therapy services at highly respected senior living communities in Sauk City, Verona and/or Middleton. Requirements
- Current Wisconsin Speech-Language Pathology license and ASHA CCCs
- Excellent communication and documentation skills
- A passion for working with older adults in skilled nursing and senior living settings
- Professionalism, flexibility, and a team-oriented mindset
Responsibilities
- Evaluate and treat adult and geriatric residents with communication, cognitive, and swallowing disorders
- Develop and implement individualized, patient-centered treatment plans
- Collaborate with interdisciplinary team members to enhance quality of life and function
- Accurately document services per company and regulatory standards
