About The Position

The Speech Language Pathologist PRN at the TX - SNF - Swingbed Facility at Christus in Jacksonville plays a critical role in assessing, diagnosing, and treating speech, language, cognitive-communication, and swallowing disorders in patients across a skilled nursing and swing bed setting. This position requires delivering individualized therapy plans that promote patient recovery and enhance communication abilities, thereby improving overall quality of life. The role involves collaborating closely with interdisciplinary healthcare teams to ensure comprehensive care and continuity for patients transitioning between care levels. The Speech Language Pathologist will also provide education and support to patients and their families, empowering them with strategies to manage communication and swallowing challenges. Ultimately, this position contributes to the facility’s mission of delivering compassionate, patient-centered care through expert clinical practice and ongoing professional development.

Requirements

  • Valid Speech Language Pathologist License
  • CPR certification
  • Valid Driver’s License
  • Valid Auto Insurance
  • Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
  • Must be able to lift, carry, pull, or push up to 100 lbs.

Nice To Haves

  • 1 year of rehab experience preferred
  • Long Term Care experience preferred
  • Vital-Stim certification preferred
  • HomeCare HomeBase experience preferred

Responsibilities

  • Administer and evaluate hearing or speech and language test results, barium swallow results, or medical or background information to diagnose and plan treatment for speech, language, fluency, voice, or swallowing disorders.
  • Develop or implement treatment plans for problems such as stuttering, delayed language, swallowing disorders, or inappropriate pitch or harsh voice problems, based on own assessments and recommendations of physicians, psychologists, or social workers.
  • Participate in and write reports for meetings regarding patients' progress, such as individualized educational planning (IEP) meetings, in-service meetings, or intervention assistance team meetings.
  • Instruct clients in techniques for more effective communication, such as sign language, lip reading, or voice improvement.
  • Educate patients and family members about various topics, such as communication techniques or strategies to cope with or to avoid personal misunderstandings.
  • Develop speech exercise programs to reduce disabilities.
  • Teach clients to control or strengthen tongue, jaw, face muscles, or breathing mechanisms.
  • Supervise or collaborate with the therapy team.
