About The Position

East Bay Post Acute is seeking a licensed Speech Language Pathologist to join our in-house therapy program. The successful candidate will be responsible for the evaluation, diagnosis, and treatment of communication and swallowing disorders for our resident population. The role requires a highly organized professional who can effectively collaborate with a multidisciplinary team. Requirements

  • Current and valid Speech-Language Pathologist license.
  • Ability to communicate positively and effectively with patients, families, and professional colleagues.
  • Strong organizational and documentation skills.

Responsibilities

  • Clinical Assessments: Develop and gain approval for effective treatment plans based on physician referrals.
  • Patient Care: Treat patients in accordance with the prescribed plan, adapting as needed and documenting all changes.
  • Interdisciplinary Communication: Participate in conferences and meetings to coordinate and review patient care plans with the health team.
  • Staff Training: Provide in-service training for facility staff and act as a clinical preceptor for CFY/RFP and student programs.
  • Documentation: Ensure all evaluations, treatment notes, and recertification documentation are complete and compliant with state and payer regulations.
  • Discharge Planning: Instruct families and nursing staff on maintenance programs to prepare patients for discharge from therapy services.
