We are currently seeking a Speech-Language Pathologist to join our dedicated team. Our facility is conveniently located off El Camino Real (Hwy 82) and the 380 freeway in San Bruno, California. Requirements

  • Licensed Speech-Language Pathologist

Responsibilities

  • Develop effective treatment plans and obtain necessary approvals from referring physicians.
  • Provide patient care according to the physician-approved treatment plan.
  • Communicate regularly with supervisors and other healthcare team members regarding patient progress, challenges, and plans.
  • Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  • Engage in in-service training programs for facility staff.
  • Maintain accurate documentation of evaluations, daily treatment notes, weekly progress reports, and recertification documentation in accordance with the Speech and Language Pathology Board State Practice Act and regulatory requirements.
  • Document treatment changes per facility policies and procedures.
  • Educate patients’ families or nursing staff on maintenance programs and provide caregiver training in preparation for discharge.
  • Participate in discharge planning to ensure a smooth transition for patients.
  • Serve as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologists and student programs.
  • Comply with the Speech-Language Pathology and Audiology Bureau State Practice Act and facility policies.
  • Build positive, effective relationships with patients, residents, families, staff, and professional colleagues.
