About The Position
Midtown Oaks Post Acute is seeking a Speech-Language Pathologist (SLP) to join our skilled nursing/post-acute care team. We’re looking for a compassionate, motivated clinician who is passionate about helping patients improve communication, cognition, and swallowing function. Requirements
- Active California Speech-Language Pathologist license (or CFY candidates welcome)
- Skilled nursing or post-acute experience preferred, but not required
- Strong communication and documentation skills
- Compassionate, team-oriented approach to care

Responsibilities
- Evaluate and treat patients with speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans
- Perform dysphagia assessments and interventions
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Collaborate with nursing, dietary, therapy, and interdisciplinary teams
- Educate patients and families on treatment plans and compensatory strategies
