About The Position

Midtown Oaks Post Acute is seeking a Speech-Language Pathologist (SLP) to join our skilled nursing/post-acute care team. We’re looking for a compassionate, motivated clinician who is passionate about helping patients improve communication, cognition, and swallowing function. Requirements

  • Active California Speech-Language Pathologist license (or CFY candidates welcome)
  • Skilled nursing or post-acute experience preferred, but not required
  • Strong communication and documentation skills
  • Compassionate, team-oriented approach to care

Responsibilities

  • Evaluate and treat patients with speech, language, cognitive, and swallowing disorders
  • Develop and implement individualized treatment plans
  • Perform dysphagia assessments and interventions
  • Document evaluations, progress notes, and discharge summaries accurately and timely
  • Collaborate with nursing, dietary, therapy, and interdisciplinary teams
  • Educate patients and families on treatment plans and compensatory strategies
