Requirements

Master’s or Doctoral degree in Speech-Language Pathology
Current California SLP license (or eligibility) in good standing
Strong communication and organizational skills
Ability to thrive in a fast-paced, team-driven environment
Experience in skilled nursing or post-acute care preferred
Maintain compliance with state practice acts and facility policies
Meet continuing education requirements

Nice To Haves

Proficiency with EMR systems (e.g.,Net Health, PointClickCare, Casamba) is a plus

Responsibilities

Evaluate patients within 24 hours (or 1 business day) of referral
Develop and implement individualized treatment plans
Provide therapy services in accordance with physician orders
Supervise Speech-Language Pathologist Assistants as applicable
Collaborate with interdisciplinary teams on patient progress and care plans
Participate in care conferences, utilization review, and rehab meetings
Complete accurate and timely documentation (evaluations, daily notes, progress reports)
Educate patients, families, and staff on therapy plans and discharge needs
Assist with discharge planning and caregiver training
Provide in-service training and support ongoing staff development
