Organizes and conducts Speech-Language Pathology services to facilitate the (re)habilitation of people with physical and/or psychosocial disabilities. The Speech-Language Pathologist directs the patient's participation in selected activities/treatments to promote and/or maintain good health and communication. Organizes and conducts the Speech-Language Pathology plan of care to facilitate the habilitation or rehabilitation of people with speech, language, cognitive-linguistic, psychosocial, and swallowing disabilities. Directs patient participation in selected activities to establish, enhance, and restore speech, language, cognitive-linguistic, psychosocial, and swallowing abilities. Performs evaluation(s), develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum, initiates and participates in the discharge planning process. Educates patient/family/caregiver in applicable health information, treatment and prevention. Performs other department activities such as, but not limited to, documentation, billing, support of department operations, etc. Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Monday-Friday. This position is part time 20 hours per week. The territory will be for but not limited to Lorain and Western Cuyahoga counties and surrounding area. The population is pediatrics. 100%25 travel to the patients homes. This will have 15 visits per week.
