About The Position

FOX Rehabilitation is seeking passionate, licensed Speech Language Pathologists in Oak Creek, WI, to provide exceptional one-on-one care to older adults. This role involves delivering therapy in private residences or Senior Living Communities, utilizing FOX's Geriatric House Calls™ model to improve patients' mobility, independence, and quality of life. Clinicians have autonomy and flexibility to create and manage their own treatment schedules, build lasting patient relationships, and may incorporate telehealth for greater flexibility and continuity of care. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and abolishing ageism, offering a supportive, interdisciplinary environment where clinicians are valued and empowered to make decisions and grow professionally. The company offers various employment types including full-time, part-time, and PRN/FOX Flex roles.

Requirements

  • Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain
  • Degree from an accredited Speech Language Pathology program
  • Basic computer and EMR proficiency
  • Current CPR certification

Responsibilities

  • Provide one-on-one Speech Language Pathology to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
  • Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
  • Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
  • Document effectively within FOX’s EMR system in accordance with regulatory standards
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
  • Provide telehealth / virtual care when appropriate to extend access and flexibility
