About The Position

Major Responsibilities: Patient Assessments / Reassessments  Performs appropriate patient assessments resulting in an effective treatment plan.  Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated.  Involves the patient and /or family in the assessment process and the formation of the care plan. Care Planning / Interdisciplinary Communication  Provides care in accordance with physician’s orders.  Demonstrates appropriate case management to ensure quality of care and best patient outcome.  Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. Patient Outcomes  Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne). Speech Therapy Modalities / Patient Treatment  Demonstrates knowledge of speech therapy modalities / treatments and performs correctly. Operates Equipment Safely and Correctly  Demonstrates knowledge of and competency in using equipment pertinent to providing speech therapy services. Patient Education  Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning.  Includes the family in teaching as appropriate. Documentation  Documentation meets current standards and policies.  Documentation is legible.  No inappropriate abbreviations are used.  Documentation is completed in a timely manner.  Medical Records are closed as per policy. Charge Capture  Time is adequately documented in the assessments and daily treatment notes.  Each charge bill adequately reflects all services provided. Demonstrates self-growth as a professional therapist.  Engages in continuing education and professional growth opportunities.

Requirements

  • Georgia Speech and Language Pathologist License.
  • Educational type and level as required by license.
  • CFY will be considered.
  • Good interpersonal and communication skills, time management skills, and the ability to be flexible and dependable.
  • Must be able to use visual acuity to see monitor screen, computer and hard copy materials.
  • Must be able to hear and verbally communicate in person and over the phone or radio.
  • Must be able to sit for prolonged periods of time (up to 2 hours).
  • Must be able to comprehend and learn operation of various office equipment.
  • Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 50-60# specific to job evaluation.
  • Must be able to forward reach, overhead reach, bend, squat, kneel and apply proper body mechanics during the transfers and transport supplies and/or equipment using proper body mechanics.
  • Must be able to lift up to 10# specific to job evaluation.
