Work collaboratively within a multidisciplinary rehabilitation and medical team to provide direct therapy, patient and family education, and clinical recommendations to support optimal outcomes. Include thorough documentation of evaluations, treatment sessions, and patient progress, as well as ongoing communication with providers to ensure coordinated care. Contribute to admission decision-making, discharge planning, and recommendations for continued services or higher levels of care, all while navigating the unique and rewarding challenges of a rural healthcare environment.
