About The Position

As a Speech-Language Pathologist with Traditional Home Health Care, you are able to give each patient your undivided attention as you care for them. Job Title: Speech Language Pathologist (SLP), Speech Therapist Job Location: Our Speech Therapists see patients in the most ideal setting - the patient's home! Our office is located in East Stroudsburg, PA. Shift(s): Per diem schedules available. Hours of operation are Monday - Friday 8:30 - 5:00.

Requirements

Current license as a Speech Language Pathologist in Pennsylvania
Certificate of Clinical Competence (CCC)
Must have a current drivers license
Ability to travel to case assignments as assigned.
Ability to work independently with minimal supervision and manage time effectively
Strong interpersonal skills.

Responsibilities

Evaluate visit and assess patient care and plans with patient/family and physician.
Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence
Teaches and supervises patient and family in the home environment regarding treatment, self-care, coping and preventive health measures.
Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (i.e., social services, therapists, etc.) to carry out treatment plan objectives.
