About The Position
CareOne Home Health is seeking a Speech Language Pathologist to add to our growing team of highly-regarded health professionals. The Speech Language Pathologist is responsible for speech therapy services for patients in San Joaquin County and surrounding communities. This is a part time or per diem opportunity.
Requirements
- Possesses a master’s or doctoral degree in speech-language pathology (SLP), and licensed speech language pathologist in the state of California.
- Minimum 1 year(s) experience as a working SLP
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- Possesses and maintains current CPR certification
Nice To Haves
- 1 year home health experience preferred
- Experience with Kinnser (WellSky) EMR preferred
- Demonstrates good verbal and written communication, and organization skills
Responsibilities
- Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
- Ensuring coordination of care with other team members and marketing team
- Identifies functional speech defects and establishes a plan of care to improve patient’s function.
- Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
- May supervise home health aide in procedures directly related to the speech–language therapy plan of care
- Performs other duties as required to facilitate the delivery of speech language pathology services.
