About The Position

Speech-Language Pathologist (SLP) – Home Health (Per Diem) Location: Serving patients in their homes throughout the Mahoning Valley Status: Per Diem / PRN Provide Life-Changing Care, Right Where It Matters Most. Southwoods Health is seeking a compassionate and skilled Per Diem Speech Therapist to join our growing Home Health team. In this role, you won't just provide therapy—you will help patients regain their independence and quality of life in the comfort of their own homes. Your Impact as a Home Health SLP: Patient-Centered Care: Conduct comprehensive evaluations and administer medically prescribed treatments to help patients overcome communication, cognitive, or swallowing disorders. Clinical Collaboration: Partner with physicians and multidisciplinary teams to develop and adapt treatment plans based on patient progress and medical necessity. Family Education: Empower patients and their families by providing essential education on recovery processes, physician protocols, and home exercise programs. Documentation Excellence: Maintain accurate clinical records in accordance with state, federal, and Southwoods standards, ensuring timely reporting of patient outcomes. Mentorship: Provide supervision for students and support personnel in alignment with Ohio licensure requirements. What You’ll Need to Succeed: Education: Graduate of an accredited Speech-Language Pathology program. Licensure: Active Ohio State Licensure as a Speech Therapist. Certification: Current American Heart Association (AHA) Basic Life Support (BLS) certification. Experience: Prior experience in Home Health therapy is highly preferred. Requirements

  • Graduate of an accredited Speech-Language Pathology program.
  • Active Ohio State Licensure as a Speech Therapist.
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Strong interpersonal communication, clinical problem-solving, and the ability to work independently in a home-care setting.
