About The Position

A Skilled Nursing Facility is seeking a licensed Speech-Language Pathologist (SLP) to provide therapy services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting. The SLP will evaluate, treat, and support residents with speech, cognitive, and swallowing disorders while ensuring regulatory-compliant documentation and interdisciplinary collaboration.

Requirements

  • Active Speech-Language Pathologist (SLP) license in the applicable state
  • Certificate of Clinical Competence (CCC-SLP) preferred or required (depending on facility)
  • Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
  • Strong understanding of skilled documentation and Medicare Part A / Part B guidelines
  • Experience with dysphagia management and cognitive therapy interventions
  • Ability to manage a productive caseload in a SNF environment

Responsibilities

  • Evaluate residents and develop individualized speech therapy treatment plans
  • Provide therapy for dysphagia, communication disorders, and cognitive impairments
  • Perform bedside swallowing evaluations and implement appropriate interventions
  • Monitor resident progress and adjust treatment plans as needed
  • Maintain accurate and compliant documentation in accordance with Medicare guidelines
  • Participate in interdisciplinary care plan meetings and discharge planning
  • Collaborate with nursing, rehab, and medical teams to support resident outcomes
