About The Position

A Skilled Nursing Facility in Hobe Sound, FL is seeking a licensed Speech-Language Pathologist (SLP) to provide evaluation and treatment services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting. The SLP will work with medically complex residents, focusing on dysphagia management, cognitive-communication therapy, and functional speech interventions.

Requirements

  • Active Florida Speech-Language Pathologist license required
  • Minimum 1+ year of experience in a Skilled Nursing Facility or post-acute rehab setting required
  • Strong experience with dysphagia management
  • Working knowledge of Medicare Part A and Part B documentation standards
  • Ability to manage a productive caseload in a SNF environment
  • Strong interdisciplinary communication skills

Responsibilities

  • Evaluate residents for speech, language, cognitive, and swallowing disorders
  • Develop and implement individualized treatment plans
  • Provide dysphagia management and perform bedside swallowing evaluations
  • Deliver cognitive-communication and speech/language therapy
  • Maintain accurate and compliant documentation in accordance with Medicare guidelines
  • Participate in interdisciplinary care plan meetings
  • Support safe discharge planning and continuity of care
  • Collaborate with nursing, dietary, and rehab teams
