About The Position

Speech Language Pathologists (SLP) Constellation Health Services is seeking a Speech Language Pathologists (SLP) to join our Home Care team! This a field based position, where the SLP will be responsible for the implementation of standards of care for speech language pathology services and for adherence to the written plan of care established by the physician. Requirements

  • Licensed and Registered Speech Language Pathologist (SLP) in the state of employment.
  • At least 1 year of clinical experience; Home Care experience preferred.
  • Oasis experience strongly desired.
  • Have reliable transportation and willing to commute to provide the best in-home patient care.
  • Willing to take advantage of our excellent training system comprised of in-house and field orientation and become an elite healthcare provider!
  • Basic knowledge of iOs operating system.

Responsibilities

  • Conducts appropriate evaluative procedures, including swallowing evaluations upon physician's referral, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.
  • Identifies speech defects and establishes goals and plan of care to improve patient's function and participates in the development of the plan of care.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system.
  • Submits all clinical documentation, time records and other information in the required time frame.
