About The Position

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) for Okaloosa County and surrounding areas. This role offers significant flexibility, the opportunity to build meaningful patient relationships, and control over income through a pay-per-visit model. The position focuses on providing 1:1 patient care in their homes, with territories strategically designed to minimize drive time. Therapists will benefit from autonomy in managing their schedules, with no required weekends or holidays. The company supports its clinicians with technology like AI-assisted charting to reduce documentation time, user-friendly EMR (WellSky/Kinnser), structured onboarding, mentorship, ongoing training, free CEUs, and clear career advancement opportunities. Employment options include PRN or 1099 contractor roles.

Requirements

  • Active, unencumbered Florida Speech-Language Pathologist license
  • Valid driver’s license, reliable transportation, and auto insurance

  • OASIS/Medicare documentation experience

Responsibilities

  • Conduct speech, language, cognitive, and swallowing evaluations
  • Develop individualized treatment plans based on physician orders
  • Provide therapy for communication, cognition, and dysphagia
  • Educate patients and caregivers to improve functional outcomes
  • Collaborate with physicians and interdisciplinary teams
  • Complete OASIS documentation as required
  • Document visits using streamlined, AI-assisted technology
