About The Position

The Speech Language Pathologist (SLP) Full time Salary 95k W/retention 5k. 28 visit quota weekly Hours: AM/PM/Evening (after school patients) Position Responsibilities Summary: Speech pathologists work with people who cannot speak or have difficulty speaking; have speech problems, such as stuttering, an unwanted accent, or an inappropriate pitch; have problems understanding language; or have communication impairments, such as attention, memory, and problem-solving disorders. They also work with people who have trouble swallowing. Speech pathologists' work involves assessment, diagnosis, treatment and prevention of speech-related disorders.

Requirements

  • Licensed as a Speech Language Pathologist (SLP) in TX
  • Must be able to provide adult and pediatric services to include swallowing technique.

Responsibilities

  • Provide guidance and clinical oversight for agency Home Health SLP Assistants (SLPA)
  • Provide speech therapy services to client according to a physician’s written orders.
  • Assess and evaluate therapeutic/rehabilitative/functional status.
  • Develop a plan of care and treatment making sure plan of care is consistent with assessment;
  • Participate in the development of the client’s total plan of treatment;
  • Identify, develop or adapt work and/or daily activities that enhance the client’s quality of life;
  • Assess the home environment especially as it relates to fall risk and interventions and home safety.
  • Report client’s response to treatment or changes in condition to the Director of Nursing and physician.
  • As appropriate, coordinate interventions with other health care providers and live settings such as childcare center, preschool, school, worksite, adult day care or residential setting
  • Instruct other personnel and/or family members and caregivers on the intervention goals.
  • Assist in identifying client and family/caregiver needs for other services.
  • Prepare and submit clinical and progress summaries based on identified and attained goals.
  • Participate in discharge planning, as appropriate.
  • Assure compliance with all local, state and federal laws and regulations.
