About The Position

The Speech Language Pathologist (SLP) at Mercyhealth at Home specializes in assessment and diagnosis; intervention, management, counseling, and follow-up services for disorders of speech, language, cognitive aspects of communication, swallowing, or other upper aero digestive functions. This position provides services in a home setting and involves a variety of responsibilities including reviewing physician referrals, evaluating patients, planning and conducting rehabilitative treatment programs, and maintaining thorough documentation. The SLP will also provide counseling and guidance, develop individualized treatment plans, and collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

  • Licensed Speech Therapy in the State of Illinois.
  • Approved CPR or BLS certification required within 90 days of hire.
  • Maintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.
  • Have and maintain an acceptable driving record per Mercy's HR-01 Fleet Safety policy.

  • Home health experience preferred.

Responsibilities

  • Reviews Physician's referral, patient's condition and medical records to determine speech therapy treatment required.
  • Obtains history and subjective components of the evaluation.
  • Diagnoses and evaluates speech, language, communicative, cognitive, and swallowing skills in the home setting.
  • Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency.
  • Provides counseling and guidance and language development therapy to patients.
  • Reviews individual file to obtain background information prior to evaluation.
  • Administers, scores, and interprets specialized hearing, speech and language tests.
  • Develops and implements individualized plans for assigned clients.
  • Reviews treatment plan, and assesses individual performance to modify or write new programs.
  • Evaluates swallowing abilities via bedside swallow/video studies and develops a plan of care.
  • Maintains records as required by law and establishment's policy.
  • Performs complete, thorough, and timely electronic documentation.
  • Instructs individuals to monitor their own speech and practice new skills.
  • Acts as consultant to educational, medical, and other professional groups.
  • Confers with Physician and other practitioners to obtain additional patient information and suggest revisions in treatment program.
  • Participates in the PI and CQI process through committees as needed.
  • Assists in the orientation and training of new staff.
