About The Position

Empath Home Health, part of Empath Health's not-for-profit network, is seeking a PRN Home Health Speech Language Pathologist (SLP) to deliver skilled, patient-centered care in the home setting. This role supports patients in the Naples area. This PRN opportunity offers flexibility and the chance to make a meaningful impact by helping patients improve communication, swallowing, and cognitive function. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care. Requirements

  • Graduate of an accredited program meeting certification requirements for ASHA, or actively progressing toward certification
  • Current state licensure as a Speech Language Pathologist
  • One (1) year of home health experience preferred
  • Strong clinical assessment, communication, and patient-education skills

Responsibilities

  • Provide skilled speech therapy services in accordance with physician orders, the plan of care, and regulatory standards
  • Complete initial and ongoing patient assessments, including required OASIS assessments
  • Develop individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Educate patients and families on swallowing safety, dietary consistency, and communication strategies
  • Support cognitive, sensory-motor, language, and memory development through individualized therapeutic interventions
