About The Position

Pacific Coast Post Acute (PCPA) is a short-term rehabilitation and skilled nursing facility located in beautiful Monterey County, just 30 minutes from the Monterey Bay. As a premiere teaching facility for post-acute care, PCPA offers a dynamic environment with exposure to memory care and a growing outpatient program. Our state-of-the-art facility features one of the largest gyms in a post-acute setting, including a dedicated ADL area with a functional kitchen, washer/dryer, and tub/shower combination.

Requirements

• Current state licensure or in the process of obtaining appropriate state licensure.

Responsibilities

• Conduct comprehensive assessments and evaluations to identify speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs and goals.
• Provide skilled therapeutic interventions for speech, language, voice, and fluency disorders.
• Manage and treat dysphagia (swallowing disorders), including providing appropriate therapy and recommending diet modifications.
• Document resident progress accurately and in a timely manner, and communicate effectively with the interdisciplinary team.
• Educate residents, families, and staff on communication strategies and compensatory techniques.
• Collaborate with Occupational Therapists, Physical Therapists, and other healthcare professionals to ensure coordinated and holistic patient care.
• Participate in program development and departmental meetings.
