About The Position

We are seeking a compassionate and motivated PRN Speech Language Pathologist (SLP) to join our growing Home Health team. This flexible role is ideal for clinicians who value autonomy, meaningful one-on-one patient care, and the ability to truly impact recovery in a home-based setting. As a Home Health SLP, you will provide skilled therapy services to patients in their homes, helping them regain communication abilities, improve swallowing safety, and enhance overall quality of life.

Requirements

• Current Speech Language Pathologist (SLP) license in the state of Nevada
• Certificate of Clinical Competence (CCC-SLP)
• Strong clinical judgment and ability to work independently
• Excellent communication and patient education skills
• Reliable transportation for in-home visits
• Valid Speech Language Pathologist License
• CPR certification
• Valid Driver’s License
• Valid Auto Insurance

Nice To Haves

• Previous home health experience strongly preferred
• 1 year of rehab experience preferred
• Long Term Care experience preferred
• Vital-Stim certification preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders in the home setting
• Develop individualized care plans based on comprehensive assessments and physician collaboration
• Provide dysphagia management, including swallowing strategies and safe diet recommendations
• Implement therapy techniques to improve articulation, fluency, cognition, and functional communication
• Train patients and caregivers on communication strategies and safe swallowing techniques
• Document evaluations, progress notes, and discharge summaries in compliance with regulatory and agency standards
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care
• Adjust treatment plans based on patient progress and home environment needs
