About The Position

Physician Affiliate Group of New York (PAGNY) is adding a Speech Language Pathologist (Per Diem) to the group at NYC Health + Hospitals/South Brooklyn Health. Requirements

  • Master's Degree in Speech Language Pathology.
  • Speech Language Pathologist NYS License.
  • Certification in American Speech Language, Hearing Association Certificate of Clinic Competency, and continuing education units as required for recertification by both agencies.
  • At least one year of paid, professional experience within the last five years in the practice of speech language pathology in a hospital, health care, rehabilitative, or related facility.

  • Hospital experience is preferred.

Responsibilities

  • Screens patients to determine absence or presence of speech language, hearing, oropharyngeal/swallowing impairments requiring further evaluation and/or referral.
  • Performs complete evaluations, including case history and testing.
  • Selection and administration of formal diagnostic test(s) and interpretation of results to determine etiology, type, nature, diagnosis and severity of presenting disorder.
  • Tests stimulability and determines prognosis for improvement, makes recommendations for treatment, communicates the evaluation results to referring physicians, patients, families/others, performs reevaluations as required, and prepares written documentation of same.
  • Attends team rounds and evaluation conferences to discuss and determine progress, changes in care plan, prognosis and treatment needs, and prepares written documentation of same.
  • Develops and implements written individualized patient care plans, including long- and short-term functional goals with criteria and time frames, provides treatment to restore improve and/or augment communication and swallowing for those patients with a diagnosed communication disorder and/or dysphagia requiring treatment and/or a home program, modifies treatment plans periodically as required, provides opportunities for patient/family/other input into the care plan, and prepares written documentation of progress as required.
