Requirements

• Licensed and current registration to practice as a Speech Language Pathologist in New York State required.
• Minimum of one year post-licensure experience in rehabilitation with adult speech/language impairment populations.
• Good computer skills - Electronic Medical Record experience required.

Responsibilities

• Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for speech language pathology.
• Identify patient needs for other disciplines and services.
• Coordinate and evaluate the plan of care with all other disciplines and physicians for an assigned caseload.
• Instruct the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.
• Document visits in the EMR system.
