About The Position

Northwell Health's Hospice Care Network is seeking a dedicated Hospice Physician to join their esteemed program at the Mary Ann Tully Hospice Inn in Melville, NY. This role involves providing comprehensive end-of-life care in a supportive and professional environment, catering to patients transitioning from hospital to home or in the final stages of illness. The ideal candidate will be a Board Certified or Board Eligible physician in Internal Medicine, Family Medicine, Palliative Medicine, or Geriatric Medicine, with a preference for those trained in Geriatric and/or Palliative Medicine.

Requirements

• Board Certified or Board Eligible in Internal Medicine, Family Medicine, Palliative Medicine, or Geriatric Medicine.
• Experience in hospice or palliative care is preferred.
• Strong communication and interpersonal skills to interact with patients and families.

Nice To Haves

• Experience in a leadership role within a healthcare setting.
• Familiarity with hospice care regulations and standards.

Responsibilities

• Provide comprehensive end-of-life care to patients in a hospice setting.
• Collaborate with a multidisciplinary team to ensure high-quality patient care.
• Engage with families to provide support and guidance during the end-of-life process.
• Participate in the development and implementation of care plans for patients.
• Maintain accurate and timely medical records and documentation.
