About The Position

Clearway Health is seeking an onsite Specialty Pharmacist to join our team and play a critical role in delivering high-quality, patient-centered care. In this role, you will ensure impeccable pharmacy operations by adhering to departmental policies, procedures, and guidelines while accurately compounding, verifying, and dispensing specialty medications. You will conduct comprehensive medication reconciliation within the electronic medical record, identify potential medication-related or disease-state interactions, and complete disease-specific questionnaires. As a key clinical partner to specialty clinics, you will help identify patients who may qualify for GLP-1 therapy, facilitate provider consultations, and follow patient progress throughout therapy. Your ongoing monitoring will allow you to recognize when treatment outcomes are not effective or sustainable and collaborate closely with providers to optimize therapy plans. Serving as a direct point of contact for clinical support, your attention to detail, professionalism, and enthusiasm will help ensure patient safety, promote adherence, and make a meaningful impact on patient outcomes—one prescription at a time.

Requirements

  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
  • 2+ years of direct pharmacy experience is required
  • A comprehensive understanding of the procedures, systems, and equipment used by pharmacists and technicians in the pharmacy and an elevated level of proficiency in performing pharmacist’s duties and responsibilities.
  • A high level of analytical ability, organizational and personal management skill and focus to ensure the concerted approach to operations.
  • Practice experience working with as well as revising and/or implementing information systems.
  • Excellent interpersonal and phone communication skills.
  • Professional in conduct and demeanor.
  • Proficient with standard software used in healthcare office settings (i.e. MS Word, Excel, web browser, etc.).

Nice To Haves

  • PGY1 preferred
  • Knowledge and understanding of the federal funded 340B program is highly preferred.
  • QS1 and Specialty Pharmacy experience is preferred.
  • Depending on site-specific requirements, an active certification in good standing as a Certified Specialty Pharmacist (CSP) from the National Association of Specialty Pharmacy (NASP) may be required but is preferred.
  • Experience with Specialty medications and immunizations is preferred.

Responsibilities

  • Oversee the accurate preparation of medications, ensuring precision and attention to every patient detail for the highest quality prescriptions.
  • Verify all medications before they leave the pharmacy, maintaining strict quality standards.
  • Compound prescribed medications, package and label with precision, ensuring correct and safe delivery to every patient.
  • Provide consultations to customers, serving as a trusted source for information and support.
  • Dispense medical and surgical products, demonstrating a commitment to accuracy, including handling restricted items.
  • Ensure meticulous record-keeping to ensure the highest standards of accuracy and safety for all dispensed prescriptions.
  • Handle order entry into pharmacy management systems to streamline processes for maximum efficiency.
  • Provide patient care, encompassing enrollments, ongoing adherence follow-ups, and ensuring patients receive the right care.
  • Perform medication reconciliation and collaborate with electronic medical records to ensure patient safety.
  • Provide pharmacy clinical support to specialty clinics.
  • Supervise compliance with federal, state, and applicable laws, optimizing operations through guidance and strategic reassignments.
  • Develop the pharmacy staff, providing direction, coaching, and identifying training gaps for career advancement.
  • Maintain contact with physicians and providers to ensure a seamless experience for patients.
  • Handle third-party insurance forms and paperwork to ensure smooth transactions and clear records.
  • Uphold the highest standards, providing a confidential and respectful environment for customers.
  • Ensure compliance with accreditation, legal, regulatory, and safety requirements, including DEMPOS standards.
  • Provide drug education and counseling to help patients navigate their treatments.
  • Share knowledge with healthcare practitioners through in-service education.
  • Participate in department staff development conferences and related activities.
  • Maintain professional and specialized competencies through continued education.
  • Adhere to the highest standards of performance and conduct, prioritizing patient rights to ensure the best possible customer service and patient care.
