About The Position

Job Details Interprets physicians' prescriptions and dispenses medications for high dollar specialty and other medications within regulatory, accreditation, and policy guidelines. Provides medication therapy management by actively managing drug therapy; identifies, prevents and resolves medication-related problems. (e.g. Counsels patients and/or medical staff and handles problem resolution). Completes patient records, initial assessments, medication profiles, initial fill notes and continuing progress notes. Participates in the coordination of care to meet patient outcomes and goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services. Interacts with internal and external clinical and non-clinical personnel to support benefits investigation, financial assistance and prior authorization needs. Supports activities for mitigating patient side effects and promoting therapy adherence. Engages in practice pull through for specialty and other orders. Aids in development and maintenance of patient management program. Participates and undertakes initiatives and projects impacting critical clinical, operational and financial areas. Job Description ESSENTIAL FUNCTIONS: Applies practical knowledge of job area typically obtained through advanced education, specialty pharmacy certification, or work experience. Works independently with general supervision. Problems faced are difficult but typically not complex. May influence others within the job area through explanation of facts, policies and practices. Coaching and reviewing the work of technical staff. Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson. OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions: Documents any and all interactions with patients and prescribers in the appropriate systems in compliance with protocol. Performs On-Call activities as required. Adheres to all pharmacy, state and federal laws and regulations. Performs other duties as assigned.

Requirements

  • PharmD degree with at least 3 years of Pharmacy experience or 1-2 years of Specialty Pharmacy experience is preferred.
  • Valid Pharmacist licensure in the State of Pennsylvania

  • Prior specialty pharmacy experience or residency preferred
