About The Position

The Specialized Home Direct Support Professional role involves providing essential support to individuals with disabilities, enabling them to lead self-directed lives and develop skills that enhance their quality of life. Requirements

• Must be 18 years of age and able to read and write effectively.
• High school diploma or GED required.
• Valid driver's license with an acceptable driving record is necessary.
• Must meet state and local requirements regarding background checks and training.
• Effective verbal and written communication skills are essential.
• Strong organizational skills to manage workloads and interruptions.
• Interpersonal skills to establish cooperative working relationships.
• Ability to maintain confidentiality in all matters.

Nice To Haves

• Experience working with individuals with disabilities.
• Knowledge of community resources and support services.
• First Aid and CPR certification.

Responsibilities

• Support individuals with personal care and daily routines according to their Individual Support Plan (ISP).
• Practice Support Solutions philosophy in interactions with individuals and community members.
• Assist individuals in becoming integrated into the community by providing person-centered supports.
• Ensure safe transport of individuals to events, job sites, and medical appointments.
• Manage and utilize agency resources responsibly, including personal funds and supplies.
• Attend all assigned training meetings.
• Prepare and provide meals based on scheduled menus.
• Teach daily living skills to promote independence.
• Document progress towards meeting individual outcomes.
• Utilize electronic records systems for daily shift notes and documentation.
• Ensure customer satisfaction by following Customer Service Standards in all interactions.
• Oversee the storage and administration of medications.
• Assist individuals with personal grooming and household tasks as desired.
• Maintain confidentiality of individuals supported.
