Requirements

  • High School Diploma or equivalent required.
  • 0-3 years of experience required.
  • Requires basic computer skills.
  • Requires knowledge of medical, community, and social resources.

Nice To Haves

  • Ability to lift 20-25lbs
  • Bi-Lingual a plus.

Responsibilities

  • Serves as a bridge between the community and the health care, government and social service systems.
  • Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information.
  • Provides informal counseling, support, and follow-up.
  • Most of the outreach to patients occurs telephonically and there are times when Community Health Specialists connect in-person with patients to deliver items and provide education.
  • Community Health Specialists are also required to participate in team meetings/retreats, which are in-person.
  • Community Health Specialists will be required to work in an in-office setting and must have reliable transportation to conduct community visits.
  • Community Health Specialists are also required to be available for community events that take place during nontraditional work hours (up to 25% of time).
