About The Position

The Specialist, Clinical Pharmacist at Denver Health is responsible for providing comprehensive pharmaceutical care, including medication management, patient education, and collaboration with healthcare providers. This role requires expertise in a specialized pharmacy area and involves developing policies and procedures to enhance patient care and safety.

Requirements

  • Doctorate Degree PharmD (preferred) or Bachelor's Degree BPharm required.
  • Colorado Pharmacist License required.
  • 4-6 years PGY2 residency or 4 years minimum experience in area of specialty required.
  • PHA-Licensed Pharmacist - BPS - Board of Pharmacy Specialties required.
  • BCPS-Board Certified Pharmacotherapy Specialist - BPS required.

Responsibilities

  • Interprets and clarifies orders for all types of medications.
  • Researches and responds promptly to drug information requests.
  • Monitors drug therapies for appropriateness and formulary adherence in collaboration with providers.
  • Designs therapeutic drug regimens based on patient-specific information.
  • Monitors drug therapy for potential interactions.
  • Advises patients, family, and medical staff on medication use.
  • Supervises pharmacy technicians and support staff as required.
  • Prepares or oversees the compounding, labeling, and packaging of medications.
  • Maintains secure storage and records of all medications.
  • Acts as a consultant on medication-related issues and coordinates drug utilization programs.
  • Assists in the development and maintenance of policies & procedures.
  • Completes competency standards specific to patient populations served.
  • Trains pharmacists, pharmacy residents, interns, and technicians.
  • Participates in departmental meetings and identifies opportunities for improvement.
  • Identifies and implements recommendations for process improvement.
  • Resolves medication and patient safety problems.
  • Assists in the development of programs to meet quality metrics.
  • Collects and analyzes data to monitor effectiveness of improvement work.
  • Completes all competency standards and identifies learning opportunities.
