About The Position

To assist individuals seeking assistance in navigating available community resources, including assessing their individual needs, searching for services in the 2-1-1 community resource database, conducting intakes as appropriate, and providing appropriate referrals. Service is provided primarily by phone but may also be provided by text message, live chat, email or in-person.

Requirements

  • Knowledge of social services or significant classes in social work required.
  • Ability to obtain the Community Resource Specialist credential from AIRS within 12-14 months after eligibility requirements for the exam are met.
  • Experience and interest in providing excellent customer service required.
  • Ability to navigate between and utilize a variety of computer applications simultaneously.
  • Flexible and adaptable to organization scheduling requirements. Work assignment will include weekdays, weeknights, weekends, and holidays.
  • Competent in MS Office products.
  • Ability to work independently with minimum supervision and as part of a high-performing team.

Responsibilities

  • Receives and responds to incoming 2-1-1 calls, texts, chats and emails from individuals, families, and community organizations seeking assistance to address a wide variety of needs.
  • Assesses their individual needs, provides appropriate referrals to available services through the 2-1-1 resource database, and documents all demographic information, unmet needs, and referrals provided.
  • Conducts screenings for a variety of community initiatives (such as pregnancy, health insurance, cancer prevention, financial empowerment) and promote preventive programs to help improve the individual’s and family’s health, education, income and overall well-being.
  • Conducts intakes for specific programs as assigned.
  • Conducts follow up with clients to see that needed services were received and to provide additional referrals.
  • Conducts advocacy for individuals experiencing difficulties or lacking abilities to make effective contacts with agencies.
