About The Position

Helping our community’s most vulnerable with solutions that permanently improve lives. Our Refugee program assists individuals that have been granted refugee status. We assist refugees who are new to our country to quickly gain independence and become productive members in their new community. JOB SUMMARY: Supports and coordinates the Intense Case Management (ICM) service delivery for vulnerable refugee/refugee beneficiary clients with urgent and chronic social, medical, and behavioral health conditions; directly supports clients to access, navigate, and understand the U.S. Healthcare System and other social service systems; Serves as a liaison between providers to ensure appropriate collaboration and information sharing occurs; advocates for client needs with providers; provides support to providers regarding client needs, relevant cultural and linguistic preferences and practices.

Requirements

  • Education /Background: Bachelor’s degree plus four years of related professional and/or volunteer experience, one year with individuals chronic medical or behavioral health conditions.
  • Must be able to relate well to a wide variety of groups and people in community.
  • Must have good communication skills (verbal and written).
  • Must be able to work under pressure and with varying deadlines.
  • Must be sensitive to cultural differences within the agency and community.
  • Fluent in Microsoft Office Applications (Word & Outlook).
  • Must be able to work in a fast paced environment with moderate interruptions.
  • Must be able to stoop, bend, squat, and ambulate over uneven floor surfaces including stairs and steps.
  • Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
  • You must be 21 years of age or older to drive on behalf of Catholic Charities.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
  • Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.

Responsibilities

  • Provides services as described in the Office of Refugee Resettlement (ORR) Preferred Communities Guidelines
  • Conducts a personalized intake and assessment of each client referred for services; ensures client is eligible for services and identifies vulnerability categories.
  • Collaborates with client to develop short-term and long-term goals within the scope of social, medical and behavioral health services within each client’s Service Plan.
  • Partners with client to complete tasks that support client to complete goals; actively participates in coordinated case staffing and communication of client progress, needs, and challenges.
  • Provides at minimum, monthly contact with client; documents client progress towards tasks and goals; updates client service plans at least quarterly to document client progress; provides updates at minimum quarterly to community partners as appropriate.
  • Assist clients and attend all required medical and behavioral health appointments; provides transportation as necessary for client to access services, assists clients to obtain and understand prescribed medication, dosage, and side effects; assists clients with medical benefit and medication renewals; ensures all applications for medical benefits are complete, accurate, and submitted in a timely manner.
  • Performs as liaison between refugees and community agencies by conducting regular outreach and maintaining good working relationships.
  • Works closely with other refugee service providers within and outside of Catholic Charities Community Services “CCCS” to assure appropriateness and coordination of services delivered to vulnerable refugees with intense case management needs by maintaining ongoing communication, exchange of information; conducts case staffing as needed.
  • Ensures services provided to client are individualized, and take into account client’s lived experience, trauma history, as well as linguistic and cultural needs.
  • Assists refugees in the resolution of interpersonal and family problems by providing short-term counseling and support during a crisis, provides follow up services and oversight and coordination of referrals as needed.
  • Provides specialized services for clients in need of guardianship of a minor or adult with guardianship process; provides a comprehensive overview of concept of legal guardianship, its responsibilities, and process.
  • Ensures clients who are diagnosed with HIV have a separate case file stored in a locked, secure location; ensures clients receive individualized services in relation to their diagnosis; supports clients in understanding their rights to confidentiality, follows all agency HIPAAA requirements in regards to personal health information (PHI).
  • Refers cases for behavioral health services by: assessing client's need for counseling; assisting the bilingual caseworker in determining mental health needs; judging which information is necessary to share with supervisor; communicating with mental health/Survivor of Torture professionals to ensure client benefits from services.
  • Ensures all electronic case file documentation is maintained in an accurate and up to date fashion as defined in program Scope of Work -and that all federal, state, and agency regulations are observed regarding confidentiality and case documentation management.
  • Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
  • Actively seeks out and advocates for ongoing training in relation to available community resources, ethnic community based organizations; continually attends and receives training on current refugee population trends, needs, and research-based service standards and delivery; continually builds and maintains cultural awareness in working with refugees.
