Job Description: The Transition Coordinator is responsible for the overarching management of transition supports and programs offered to students ages 18-22 years old in the Southbridge Public Schools. Specifically, the Transition Coordinator will oversee the supervision and evaluation for the job Coaches. Further, the Transition Coordinator will engage in ongoing management of transition assessment tools, conduct Transition Assessments as needed, work with IEP teams to develop goals and objectives for transition, and develop comprehensive transition plans for IEPs. The Transition Coordinator acts as the liaison for outreach to families, community based organizations, and state agencies, to ensure seamless and holistic transition planning for students who are in the process of post-secondary transition planning (ages 18-22). Further, the Transition Coordinator will oversee the development, coordination, and support of individualized transition services for students with disabilities focusing on preparing students for education, employment, independent living and community participation in accordance with their IEPs and MA State Guidelines.
