The Special Education Transition Services Facilitator, under the supervision of the Chief Officer of Special Education and Student Supports, is responsible for the coordination and facilitation of transition services, including transition assessment, transition planning, interagency linkages, and family support for students with disabilities ages 14-22. The Transition Services Facilitator is a key member of the Planning and Placement Team (PPT) and helps guide discussions to ensure the development of compliant, high-quality, and measurable postsecondary goals and transition services within the IEP. The Transition Services Facilitator serves as a liaison with adult service providers, colleges, training programs, and other postsecondary partners. In addition, the Transition Services Facilitator coordinates the District's high school and transition program work-based learning experiences for students by developing job sites, training job coaches, and placing students in community-based work settings.
