About The Position

The Transition Specialist RN/LPN is responsible for initiating and completing the admission process for patients within an acute care facility. This role involves assessing patient needs for in-home services or hospice care, coordinating the delivery of these services, and acting as a liaison between the Visiting Nurse Health System and various stakeholders including facility staff, physicians, patients, and families. The specialist also collaborates with marketing initiatives and works towards achieving referral and admission retention and growth targets within a defined territory.

Requirements

  • Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing preferred, or Licensed Practical Nurse, Masters or Bachelors of Social Work degree, or Licensed Clinical Social Work.
  • Current licensure by the Georgia State Board of Nursing strongly preferred.
  • Must have current and valid driver's license.
  • 2-3 years of experience in community health nursing and/or home health care and/or hospice care and/or case management experience preferred for nursing candidates.
  • 2-3 years of experience with hospice or hospital case management and/or discharge planning strongly preferred for all candidates.
  • Demonstrates knowledge of generally accepted nursing standards and regulatory requirements and techniques of home health nursing and/or hospice care.
  • Knowledge of all company services and eligibility requirements for those services; third party coverage.
  • Clerical ability to operate a variety of office machines and competent keyboarding skills.
  • Strong multi-tasking abilities, strong communication skills, and ability to work well with physicians, case managers, social workers, patients, and co-workers.

Nice To Haves

  • Previous experience with McKesson: Horizon Homecare preferred.

Responsibilities

  • Begin and/or complete the admission process within the acute care facility.
  • Provide a written or electronic assessment of patients' needs for in-home services and/or hospice.
  • Coordinate initial home health care and/or hospice delivery.
  • Serve as a liaison for Visiting Nurse Health System to facility staff, physicians, patients, families, and other organizations.
  • Collaborate regularly with the Account Executive on marketing initiatives within the territory.
  • Achieve referral and admission retention and growth targets from referral partners in a defined territory.
  • Collaborate with Account Executives on territory and account plans.
