The Hospice Social Worker at Compassus is responsible for providing psychosocial support and counseling to patients and their families, ensuring they receive comprehensive care in line with the company's values of Compassion, Integrity, Excellence, Teamwork, and Innovation. This role involves collaborating with an interdisciplinary team to develop and implement care plans, addressing the psychosocial needs of patients, and facilitating access to community resources.

  • Explain hospice services and Medicare benefits to patients and families; obtain Informed Consent and Election of Benefits documents as requested.
  • Submit appropriate documentation and paperwork to facilities at the completion of patient visits; ensure timely documentation according to company policy.
  • Participate as a member of the interdisciplinary team (IDT), including development and implementation of the plan of care.
  • Provide psycho-social support and counseling services to the patient and family; support the involvement of external counseling resources as needed.
  • Assist in identifying the need for intervention of other IDT members.
  • Effectively communicate patient and family needs to the IDT.
  • Collaborate with the IDT to coordinate psycho-social care and support for the patient and family.
  • Maintain updated, comprehensive care plans reflecting current problems, goals, and interventions for patients.
  • Complete initial and ongoing psycho-social assessments for patients; address concerns related to financial means, insurance, living arrangements, caregiver stressors, and community resource needs.
  • Facilitate placement in long-term care as warranted and monitor adjustment.
  • Participate in care planning meetings at long-term care facilities.
  • Support and facilitate advance planning including living will/POA documents, funeral arrangements, and memorial services.
  • Minimum of Bachelor's degree in Social Work, Psychology, Sociology, or related field with one year of social work experience in a healthcare setting required.
  • Licensure if required by state rules; supervisory oversight by MSW for hospice patient care planning and counseling needs is required.
  • Master's degree in Social Work with one year of social work experience in a healthcare setting highly preferred.
  • Experience in hospice or palliative care settings.
  • Familiarity with community resources and support services for patients and families.
